CCNC Partnership with State Health Plan Could Lead to Statewide Community-Based Model

A partnership between ActiveHealth Management and Community Care of North Carolina (CCNC) is working to build a patient-centered medical home model to benefit members of the State Health Plan for Teachers and State Employees. Under the partnership, ActiveHealth provides telephonic population health management services, including disease management, case management, and wellness coaching services for SHP members. Details about the partnership were contained in a letter sent to North Carolina physicians and other health care providers on February 4, 2011.

The partnership is seen as an interim step toward a goal of a statewide community-based model in which these services are delivered locally and managed by North Carolina physicians and CCNC networks. CCNC has received national acclaim as a physician-led model for treating Medicaid patients with chronic conditions in cost-effective ways. ActiveHealth’s role includes providing technology and support services, including outreach to patients who can benefit from the program.
