Are you a healthcare professional helping patients discuss and document their end-of-life care wishes?
The first class to certify MOST (Medical Orders for Scope of Treatment) Advance Care Planning Facilitators to be held in the Triad area will be on Friday, August 26, 2011, in the Education & Counseling Center on the Hospice campus, 121 Hospice Lane, Winston-Salem. The course will prepare professionals to have meaningful conversations with patients and/or families interested in having their wishes known, honored and documented. Those who are certified will have both the skills and the comfort level to assist in the completion of the MOST form.
Registrants must complete four computer modules prior to attending the eight-hour class. Each module usually takes about one hour to complete. Modules will be opened to the first 20 who register. Another class will be taught later this fall. Any who aren’t able to attend the August course can be placed on a waiting list for the next class.
