NCMS Member Resource Center FAQ of the Week: Patient Health Snapshot

Due to a high volume of questions coming in through the Member Resource Center, the NCMS will provide a Frequently-Asked-Question (FAQ) for its members each week to make them more aware of current medical and health care issues that their colleagues are having.

Q: Does the NCMS offer a resource that I can provide to my patients to help them document their medical conditions? This will help me more efficiently and effectively provide care to my patients.

A: Yes, the NCMS Quality of Care and Performance Committee developed the Patient Health Snapshot, a form that we recommend you distribute to your patients. This will allow your patients to accurately list their past and current medical conditions, allergies to medications, and prescription medication history. This information will also allow you, as physicians, to provide better care to your patients, especially in emergency situations.
