Pharmacy Student Survey: What Resources Influence Prescribing among Geriatric Patients?

Denise Dong is a Pharm.D student, class of 2013, at the University of North Carolina, Eshelman School of Pharmacy. She is conducting a research survey to assess what resources influence prescribing among geriatric patients.  Through this research she hopes to gain a better understanding of how to improve safety of medication-use in geriatric patients.  The survey below will help her in her research.  All information is confidential and responses will only be available to the primary investigators; survey results will not report specific information about the individual or the institution.  The survey is voluntary and will be open until January 30th, 2013.  If you have any questions please contact Denise Dong
