Tarwater responded that nationwide we are going to need to provide care for more than 30 million more people. Where are we going to put these people? He would argue that many safety nets across the country are already seeing these patients, these may be emergency rooms but the pateints are still being seen. NC  may be okay facilities wise but we are definitely going to have to rethink how we provide care.  New models of care must be carried out within the walls of those facilities.
